The Fallen is based on the saber you build/get in KotOR.

I need to catch up on KOTOR, but I think when Revan's dual-wielding (was this a boss fight?), Fallen is his red saber, while the tri-clawed one his his purple.

Revan doesn't actually dual wield in any KotOR cutscenes. I believe something like that happens in KotOR II, but I haven't gotten that far yet, and all the research I've done indicates that the Reborn design wasn't introduced until SWTOR.

In Kotor II, during a vision, you actually fight Revan wielding a red and a purple saber, although the hilt of the purple saber is not well defined, looking just like the Fallen as well. It is said to be the Reborn, but well, we don't have any proof of that!

In SWTOR: SOR, during the final fight, Revan's wielding both the Fallen and the Reborn!

You know, Darth-Revan, it has just occured to me that although you call yourself 'Revan' you don't actually own either saber. Is it because you simply haven't gotten them yet and intend to, or something else, if you don't mind me asking?

EDIT: It has been observed that our four new saber selections have the option for an Indigo blade. I looked around just a bit, but it doesn't appear that such an option has been added for the other sabers.

See, you're wrong my friend (technically). I never intended (and I do not intend) to buy SF's versions of Revan's saber. HOWEVER, I already have Revan's first saber. Call it the Fallen if you want. I bought Vader's Vault version, the REV-N Elite, which I prefer over the fallen. I intend to do a small review of the saber in an appropriate section of the forum! All I have to show for now (being at work) is this picture:

Love the Archon. Will have to get it someday. Nice to know it's a keeper model now anyway. And the Bane as well. Of course, it doesn't make any difference to me seeing as they still don't provide a PayPal option for payment, so I still gotta wait for Etsy. At least I know that they're not going to run out of stock.
