That was basically the route I took. Seeing as I received the email for a shipping label being printed a couple days back, I emailed them and explained that I didn't think the new form that they are shipping is what I wanted since I have no way of actually testing to see if the new one will work at all before their return time is up and asked for their suggestions and input as to what to do next. Will update a bit more when I find out what's going on for sure with it.
