A lot of it just comes from what you are comfortable with. There is no exact form. Some use inspiration from the 1800s others take inspiration from movies. Just dual and use the form you can handle.
You give life, You are love. You bring light to the darkness.
You give hope, You restore every heart that is broken.

And if by form you are referring to the 7 lightsaber forms that were expanded upon in books like the Jedi Path and DK's Visual Dictonary for Attack of the Clones (like Makashi and Ataru), there aren't necessarily a concrete moveset for each of them.

After reading the descriptions of each form, I decided that "my" form would be Niman (form VI) based on the fact that I like the idea of a style that incorporated force abilities more prominently, and I have entertained the idea of duel wielding.

Here are some links if you'd like to learn more about each of form:
Form I: Shii-Cho
Form II: Makashi
Form III: Soresu
Form IV: Ataru
Form V: Shien and Djem So
Form VI: Niman and its offshoot Jar'Kai
Form VII: Juyo and Vaapad
